A slide is simply moving up or down the neck whilst holding a note. Firm pressure must be maintained on the note as you slide or it will die out.

The destination note is the focus and where you begin your slide is not so important as you move off that note very quickly. Slides are a good way to end a run or riff instead of simply stopping. Chords can also be slid around the neck using the same technique.

Slides are usually played on your little finger when sliding up the neck and on the index finger when sliding down the neck.
